Below are the instructions for accessing the BCBS 1095 forms if you do not receive a postcard.

Each member will receive a BCBS postcard with instructions on accessing the 1095 forms. You must be registered with Blue Cross Blue Shield (BCBS). If you are not registered, you need to do so at  https://www.floridablue.com/

How to access the 1095-B form online

1. Log in to your member account
2. Click the down arrow next to your plan name
3. Click Tax Documents

How to request the 1095-B form by mail

1. Follow the instructions on the postcard
2. Send a written request to BCBS Customer Service at 4800 Deerwood Campus Parkway, Jacksonville, FL 32246
